Reversing Heartburn and GERDThe incidence of GERD continues to rise dramatically, but current treatments not only don. Find out how a modified Paleo diet can prevent and reverse GERD naturally. This article is part of an ongoing series comparing prescription medication with a Paleo diet as a means of treating common diseases and health problems. Click here to read the other articles in the series. Gastroesophageal Reflux Disease (GERD), a more serious form of acid reflux, is the most common digestive disorder in the United States. Sixty percent of the adult population will experience some type of GERD within a 1. The diagnosis of GERD increased by an unprecedented 2. Drugs for acid reflux and GERD are cash cows for the pharmaceutical companies. More than 6. 0 million prescriptions for GERD were filled in 2. Americans spent $1. Nexium, the most popular, brought in $5. Find out why acid- stopping drugs are a bad idea, and what to do instead. As sobering as those statistics are, it. This permits patients to self- medicate without reporting their condition to a doctor. What really causes GERD and heartburn? Instead, the prevailing scientific theory is that GERD is caused by a dysfunction of the muscular valve (sphincter) that separates the lower end of the esophagus and the stomach. This is known as the lower esophageal valve, or LES.
In GERD, the LES malfunctions because of an increase in intra- abdominal pressure. This pressure causes distention (i. But what causes the increase in abdominal pressure in the first place? Ironically, one of the main causes may be too little stomach acid, which in turn contributes to an overgrowth of bacteria in the small intestine. This idea is supported by studies on mice that have been genetically altered so that they are incapable of producing stomach acid. They develop bacterial overgrowth in their intestines. Believe it or not, stomach acid isn. The Paleo Diet is one of the hottest diet trends around. With celebrity followers and even high-end restaurants taking notice of Paleo principles. I'm pleased to introduce Jo Harding, the very talented author behind Paleo Crust, a food blog devoted to healthy low-carb & paleo lifestyle. Just like me, Jo started. Acid is in the stomach because it. It is found in all vertebrates. Such misunderstanding is perpetuated by drug companies who continue to insist that stomach acid is not essential. Meanwhile, millions of people around the world are taking acid suppressing drugs that not only fail to address the underlying causes of heartburn and GERD, but put them at risk of serious (and even life- threatening) conditions, including: Reduced absorption of essential nutrients (including B1. Increased risk of bone fractures (likely a consequence of impaired nutrient absorption)Increased bacterial overgrowth in the intestines. Decreased resistance to infections (including life- threatening ones like pneumonia and clostridium difficile)Increased risk of cancer and other diseases, such as irritable bowel syndrome (IBS), Crohn. They were never intended to be prescribed for years or even decades, as is often done today. Perhaps this is why the FDA has issued a series of reports cautioning against the prolonged use of PPIs, citing increased risk of infection, bone fractures, and life- threatening infection (clostridium difficile) as the primary concerns. This strategy actually addresses the underlying causes of the problem, whereas the conventional approach (acid- stopping drugs) merely suppresses the symptoms. In his excellent book. Norm Robillard argues. When stomach acid is sufficient and carbohydrates are consumed in moderation, they are properly broken down into glucose and rapidly absorbed in the small intestine before they can be fermented by microbes. However, if stomach acid is insufficient and/or carbohydrates are consumed in excess, some of the carbs will escape absorption and become available for intestinal microbes to ferment. The standard American diet is high in carbohydrates. For example, in one study at Duke University, patients who had failed conventional GERD treatment (i. This was true in spite of the fact that some of the patients continued to drink alcohol, smoke, and engage in other . I suggest a low- carb version of the Paleo diet for my patients with GERD, since it restricts gluten and other foods that may be problematic, in addition to reducing carbohydrate intake. A very low- carbohydrate diet is not only unnecessary for most people over the long- term, it may cause problems of its own. I. Loren Cordain’s book; my further research lead me to Chris Kresser. At the time I was readying myself for a surgical procedure for my unremitting GERD. I was taking 8. 0mg of Nexium per day and two extra- strength Tagamet pills prior to going to bed. Under the care of one of the leading Gastroenterologists from Columbia Presbyterian in NYC; I was given no alternative but to go through a drastic procedure which had a 4. Not the best odds, for sure. My other alternative would be to stay on PPIs the rest of my life (I had already taken them for 3 years); destroying my bones; and leaving me open to infections, etc. From time to time I would try to stop taking my meds but the rebound reflux was unbearable. After reading as many Paleo books as I could get my hands on and following more blogs than I can count; I started the Paleo diet removing all sources of grains, dairy, and industrial seed oils from my diet. In the meantime I gradually reduced the Nexium, etc. As the weeks went on I could feel myself getting better and better. It took me a year to get off those nasty drugs and I could not have done it without the diet. I’ve been Paleo since 2. I’ve never looked back. I’ve been able to add safe starches to my diet but never anything containing gluten (activates my reflux within hours). So what will it be for you? If your answer is Paleo, make sure to check out my book (just published in paperback with a new name: The Paleo Cure) for a detailed explanation of how to use the Paleo diet and lifestyle to prevent and reverse disease and feel better than you have in years. As always, check with your doctor before starting or stopping any new treatment plan. This is not intended to be medical advice, and is not a substitute for being under the care of a physician. How Many Calories Should I Burn a Day to Lose Weight? From my experience a lot of people hesitate to even try and lose weight because they don. They know that they should go on a diet and exercise regime but the lack of measurable goals creates uncertainty and this leads to failure before to even begin! To bypass this problem and have clear goals on what you want to achieve, you can start your weight loss process by doing some simple calorie calculations. By calculating how many calories you should be eating per day and how many calories you should be burning per day, you can have a better picture about your daily calorie needs and a rough estimate as to when you will be able to reach your ideal weight. Before getting into the mathematical part let me explain some basic weight loss concepts that will help you understand what these numbers mean. You do not have to keep a pen and paper because our daily calorie calculator will do the hard part. When do you lose weight? There are many factors that affect how much weight you can lose (and when) but in its simplest form weight loss will occur when the calories you burn are more than the calories you consume. Food and drinks (think diet) are the only sources for calories you consume while exercise, physical activity and other body factors affect the rate you burn calories. So, if you manage to control these 2 sources and create a calorie deficit you will eventually start losing weight. How many calories are in a pound? Let. In essence this figure gives you the number of calories your body needs on a daily basis in order to function properly. Note: This is not how many calories you consume per day but how many calories your body needs to function. BMR and Daily Calorie Calculator. You can use our BMR CALCULATOR above to find out your basic metabolic rate which is based on your weight, age, height and daily activity. We will use BMR to calculate how many calories you should be eating per day and then demonstrate with an example how many calories you should burn per day if you want to lose weight in a specified amount of time. Daily calorie needs to maintain current weight = BMR x Activity Factor. Note: You do not have to take a calculator to make the equation; you can use our daily calorie calculator above. Daily Activity. Activity Factor. Sedentary (little or no exercise)1. Lightly Active (light exercise, doing sports 1- 3 days per week)1. Moderately Active (sports/exercise 3- 5 days per week)1. Very Active (intense sports 6 or 7 days per week)1. Extra Active (sports and exercise, physical job or hard training)1. For example, if your BMR is 1. If you want to lose weight you should be consuming less than 2. How many calories to eat if you want to lose weight? Before getting into a real example of how to calculate the calories you should be eating per day for weight loss let me say that this is only an estimate and it is not guaranteed that by following this number you will lose or gain weight. There are many other factors involved in losing weight and the number of calories you consume is just one of the elements and since it is easy to calculate you can use it as your starting point. Let. Doing some basic calculations you can find out that 1. So in theory, if you manage to save 3. Step 4: Subtract the number of calories you need to save per day (Step 3 above) from your daily calorie needs (step 1 above)Daily Calorie needs . This is the number of calories you need to eat per day if you want to lose 5lb in 2 months! How many calories to burn per day if you want to lose weight? Since you know the number of calories you should be eating per day, you can use a calorie counter to calculate the amount of calories you actually eat (or drink) on a daily basis and find out the amount of calories you need to burn via exercise if you want to lose weight. Hint: You can use our Free Calorie Counter to keep count of your meals and activities. For example, if you are consuming. Especially at the beginning you will lose water weight and in some cases you may also lose muscle. So the results in the first few weeks may be misleading. When you make sudden or drastic changes to your diet the body will try to adapt by lowering the metabolic rate. In other words it enters a protective mode by reducing the number of calories it needs per day. As a result weight loss may slow down, here is how to lose weight and keep it off. The absolute minimum amount of calories you can consume per day is 1. It is not recommended to go beyond this number for any reason. The recommended weight to lose per week is between 1 . Anything above that can have other implications to your health and normal body functioning. Do not forget that drinks also carry calories so make sure that you include drinks in your daily calorie calculations. You can read our article on the 5 best healthy weight loss drinks that work to find out which drinks are . This means that once you calculate your daily calorie needs you can go above and below this range on different days of the week. If for example you daily kcal needs are 1. Monday and Wednesday and below this number the rest days of the week but try to avoid exaggerations. You can also read about carb cycling to access if this is something you want to try for your diet. Make sure that you give correct measurements for your weight and height when calculating your BMR. Do not lie about your activity level but try to be as accurate as possible. Do not decrease your daily calorie intake by more than 5. Better expand the time period. Reducing your intake by 5. Always consult your doctor or dietician before making any changes to your diet or exercise routine. As explained above you can use these calculations to give you an idea where you stand and how much effort you need to make until you meet your goals and target weight. Weight loss depends on a lot more factors and calculating the calories you need per day is just your starting point. Weight Loss with Medifast Exercise & Weight Loss if You're Only Eating 1,000 Calories a Day by Erin Coleman, R.D., L.D. Create an 1100 calorie deficit per day to lose 2 kilograms per week. Do you know how many calories you need to lose weight? Don't start your weight loss efforts off wrong from the start. Here's how to determine your intake. Gain 2 pounds per week; Gain 1.5 pounds per week; Gain 1 pound per week; Gain 0.5 pound per week; Maintain my current weight; Lose 0.5 pound per week. How Many Calories Do You Need to Reduce By? Studies have shown that women who eat 1000 fewer calories than they need lost around 2 pounds per week. How To Burn 3000 Calories A Day: To burn 3000 calories a day, you need to alter and eat much lesser than your current diet patterns. This means that you must adjust. Main Outcomes and Measures. Scores were measured every 3 months from baseline to 1. Secondary outcomes performed at baseline and 1. Holter, echocardiographic left atrial area, and interventricular septal thickness. Results. The intervention group showed a significantly greater reduction, compared with the control group, in weight (1. P. Additionally, there was a reduction in interventricular septal thickness in the intervention and control groups (1. P. These findings support therapy directed at weight and risk factors in the management of atrial fibrillation. Trial Registration. Inclusion criteria were symptomatic paroxysmal or persistent atrial fibrillation (in sinus rhythm at enrollment); body mass index (BMI) greater than 2. Exclusion criteria included recent participation (within 3 months) in a weight loss program; unstable international normalized ratio (INR); diabetes necessitating insulin; significant cardiac valvular disease, and inability to provide informed consent (details in e. Table 1 in Supplement). Patients were censored if they underwent ablation of atrial fibrillation or atrioventricular- node ablation. All patients provided written informed consent to the study protocol approved by the human research ethics committee of the Royal Adelaide Hospital and the University of Adelaide. Both groups underwent intensive management of cardiometabolic risk factors. A software- based obesity management system (OBEMAN) developed at the University of Adelaide was used to assess and monitor patients undergoing a tailored weight loss program. Patients were evaluated at 3- month intervals. Patient records contained generic statements without indicating group allocation. Patients were prescribed very- low- calorie meal replacement sachets (Prima Health Solutions) for 2 of their daily meals. The third meal consisted of calorie- controlled foods with high levels of animal and plant proteins and low glycemic index. A written exercise plan prescribed low- intensity exercise (walking or cycling), initially for 2. Very low- calorie meals were gradually phased out and replaced with low. Goal- directed face- to- face clinic visits were scheduled every 3 months. Participants scheduled additional visits as required and were provided 2. Participants in the intervention group were required to maintain a diet, activity, and blood pressure diary. Fish oil (3 g/d) was prescribed except for participants taking dual antiplatelet agents or oral anticoagulants. Completion of a diet and activity diary was not requested. The AFSS is a validated scale (range, 3. The secondary outcomes were 7- day Holter- derived atrial fibrillation episode and duration burden, echocardiographic left atrial area, and left ventricular wall thickness. Waist circumference was measured at the midpoint between the iliac crest and the lowest rib, and BMI was calculated. Anthropometric values were measured at 3- month intervals. Impaired glucose tolerance was managed with lifestyle measures. Metformin was added if diabetes was present. Patients with poor glycemic control (glycated hemoglobin level > 7%) were referred to a diabetes clinic. Hyperlipidemia was managed by combining lifestyle measures, HMG- Co. A (3- hydroxy- 3- methylglutaryl- coenzyme A) reductase inhibitors, and fibrates to achieve target values outlined in the National Cholesterol Education Program. Patients were asked to measure blood pressure 2 to 3 times daily at home using an appropriately sized cuff. For each recording, measurements were performed 3 times, and the mean was recorded. Hypertension was treated using renin- angiotensin- aldosterone system antagonists preferentially. Other agents were added to achieve a resting blood pressure less than 1. Hg. Ambulatory blood pressure monitoring (2. Changes in pharmacotherapy were recorded at each visit. Fasting blood was drawn at enrollment and follow- up for measurement of serum lipids, C- reactive protein (CRP), insulin, and glucose levels, using previously described methods. All patients underwent laboratory- based polysomnography, with scoring using standard criteria. Patients with moderate- severe obstructive sleep apnea (apnea- hypopnea index > 3. Those with an apnea- hypopnea index of 1. Written and verbal counseling was provided for smoking cessation and alcohol reduction. An ultimate goal of alcohol reduction (. Changes were documented at each visit. Holter recordings were obtained at baseline and repeated at 1. Analysis was performed by 2 independent scientists blinded to participant randomization. Any episode of atrial arrhythmia lasting 3. Total duration of atrial fibrillation was the cumulative sum of all discrete episodes. Left ventricular mass was calculated from the posterior wall thickness, septal wall thickness, and left ventricular end- diastolic dimension as recommended by the American Society of Echocardiography. Data were stored digitally and analyzed offline (Echo. Pac PC version 8 2. GE Health Care) by an experienced cardiologist blinded to randomization. A group with sinus rhythm maintenance had a mean baseline BMI of 3. SD, 5. 6) and a mean baseline AFSS symptom burden score of 1. SD, 7. 3); corresponding values in a group with persistent atrial fibrillation were 3. SD, 6. 0) and 1. 3. SD, 6. 6), respectively. A mean reduction of 7. Therefore, we estimated a baseline atrial fibrillation burden score of 1. In addition, with the anticipation of a worst- case- scenario 3. Eligible patients were randomized in a 1: 1 ratio to either group using SPSS version 1. IBM SPSS Inc). Continuous variables are presented as mean and 9. CI or mean and standard deviation (SD) when normally distributed on visual inspection of their histograms and as median (interquartile range) when not normally distributed. Categorical variables are summarized as count (proportion). For repeated- measures analysis of continuous dependent variables, mixed- effects modeling was used, with patient identity included as a random effect. Randomization group was entered into the mixed model as part of an interaction term with patients. Post hoc testing was performed to determine whether the group influence on the dependent variable was significant at each visit. For repeated- measures analysis of binary and nonnormal dependent variables, an analogous approach was adopted using generalized estimating equations. A binomial probability distribution with logit link function or a Poisson distribution with log link function were assumed as appropriate, as was an autoregressive correlation structure. Post hoc logistic regression was used to determine whether group allocation was predictive of subsequent need for radiofrequency catheter ablation. To account for atrial fibrillation ablations performed on participants, we performed sensitivity analyses, because these procedures may be regarded as a clinically relevant outcome and may alter the burden of atrial fibrillation in a given individual. In the first sensitivity analysis, AFSS scores and Holter data were imputed to baseline values for the time points following the ablation. In the second sensitivity analysis, these variables were imputed to the preablation value. In the third analysis, those control participants who underwent ablation of atrial fibrillation were excluded. To determine the influence of dropouts on the study findings, a further sensitivity analysis was performed. For AFSS scores, differences between final and baseline scores were regressed against patient randomization group as a predictor variable. We determined the effect on the regression coefficients of varying the missing AFSS scores in the intervention group, the control group, or both. The range of the sensitivity analysis encompassed 1 SD of the AFSS score variable. For Holter data we used a binary end point of freedom from atrial fibrillation. All statistical tests were 2- sided, and P. Analyses were performed using STATA version 1. Stata Corp). Of these, 2. The final cohort therefore included 1. Figure 1). Mean follow- up was 1. CI, 1. 2. 3- 1. 3. CI, 1. 1. 1- 1. 2. At 1. 2 months, 1. By 1. 5 months, 8. Of the 6. 9 not completing 1. One patient in the intervention group underwent atrioventricular- node ablation and pacemaker implantation at 1. Patients undergoing ablation were removed from further analysis immediately after undergoing the procedure. Baseline characteristics were similar in both groups (Table 1). Differences were evident by 3 months and persisted for the remainder of follow- up; P. No significant change was observed from baseline to 3 months in atrial fibrillation symptom burden (intervention group: 2. From 6 months to 1. Secondary analysis for AFSS symptom frequency, duration, and global episode severity scores showed significantly greater decline in the intervention group compared with the control group (group. The mean number of atrial fibrillation episodes in the intervention group decreased from 3. CI, 1. 6- 4. 9) to 0. CI, 0. 1. 9- 1. 0), and the duration of atrial fibrillation decreased from 1. CI, 7. 20- 1. 63. CI, 1. 59- 8. 22) minutes. In the control group, the mean number of atrial fibrillation episodes was 2. CI, 1. 7- 4. 0) at baseline and 2. CI, 1. 1- 3. 0) at 1. CI, 7. 85- 1. 99. CI, 7. 82- 2. 30. Both measures showed significant group differences (group. After adjustment for baseline BMI, duration of atrial fibrillation disease, longest atrial fibrillation episode, and atrial fibrillation type, the probability of having 1 or more atrial fibrillation episodes between baseline and follow- up remained lower in the intervention group (0. Height- indexed left atrial volume decreased from 3. CI, 3. 6. 9- 4. 1. Lm. Septal thickness decreased in both the intervention group (1.
